Diamond shaped bullet How does Aculert work? What is it?

n Aculert lets emergency responders use special handheld computer
   devices to access your essential medical and safety information
   instantly during emgergency treatment.

n Once you enroll in Aculert, you are sent a simple questionnaire that
   permits you to list your:

 Medical Conditions, Medications, Allergies
 Emergency contact listing - who you want called during an emergency
 Doctor's name and phone, insurance info and your hospital
 You may also provide copies of your advance directive, DNR, or other
   special notes of documents that may be important to those who
   may later treat you.

n This information is then accessible by emergency responders and
    hospital emergency rooms should it ever become necessary.

p  Emergency Responders - Personnel that are designated to aid or
        assist others and include workers assigned to fire, paramedic,

        medical workers to aid and comfort persons suffering from an accident

p  Essential Medical and Safety Information - Information you want
        emergency responders and hospitals to know should you require
        emergency care. You complete a questionnaire listing the information

p  Advanced Directive - A legal Document signed by an individual
        that outlines the level of care to be provided by medical personnel if
        the individualis unalbe to speak for himself or herself. It also may
        specify others who can make decisions on the patient's behalf.
